The insurance company pays to the dentist based on the treatment provided by him to the individuals. The premiums are payed to the insurance company by the employer.

Preferred Provider Insurance:
According to this plan, insurance is provided to specific group only, that too when the treatment is taken from particular dentists.

These insurance policies can be obtained through an insurance agent or an insurance company or by an employer or through online means, where the insurance company has all the details about the dental plans and their cost specified in their websites.
